Creating a new field report is a two step process. First, you'll need to select a health care organization to associate with the report. If your organization is in the drop down list, simply select it and fill in the rest of the fields. Once you create the report, you 'll be able to edit it later. So don't feel compelled to fill in every field or complete everything right away.

If you don't see your organization in the drop down box, you'll need to create it before you can complete the field report!

This is a critical step, because the health care organization information will be used to sort field reports for users (i.e., looking for small, rural hospitals, you can sort by these criteria). When you create the hospital report, you don't need to use the organizations real name -- although this is obviously preferred, since it helps make the report more credible and enables other users to contact the organization, if they have questions. Either way, you'll need to give the organization a name (e.g., Memorial Hospital).

You will then need to identify the city and state (this becomes part of the hospital name, so hospitals with the same name can be distinguished from each other). This is also used in sort and search processes for the field reports.

Finally, you'll need to add some demographic characteristics. Once again, this is used for sorting and searching the field reports. Click "Save Organization", and you are done (You'll be redirected to the finished report). In the near future, you will be able to edit the health care organization page (e.g., add links to the organization's website or provide additional information). When you're ready, click on the "Create a Field Report for this Organization" link, and you be taken back to the field report page.

When you return to the field report form, you will now see it in the organization in the drop down box. Fill out the rest of the form (make sure you select the performance improvement topic associated with the field report). Once you create the report, you 'll be able to edit it later. So don't feel compelled to fill in every field or complete everything right away.
