MI1 The organization transfers, at a minimum, the following information for the current episode of care:

  • Primary diagnosis
  • Secondary diagnoses/comorbidities
  • Current medications
  • Allergies
  • Short summary of the course of care provided
  • Surgical procedures, if applicable (including surgeon name, date, procedure)
  • Dietary specifications
  • Any active or suspected infections
  • Follow-up recommendations
  • Provider contact information
