MI1 The organization transfers, at a minimum, the following information for the current episode of care:
- Primary diagnosis
- Secondary diagnoses/comorbidities
- Current medications
- Allergies
- Short summary of the home care course of care provided
- Surgical procedures, if applicable (including surgeon name, date, procedure)
- Dietary specifications
- Any active or suspected infections
- Follow-up recommendations
- Provider contact information
- Weight-bearing status
- Precautions (swallowing, movement, exercise, contact precautions)
- Wound care instructions, as applicable
- Follow-up appointments required/made
